Senior Exhibition Loan Form

Carefully read the SENIOR EXHIBITION POLICY before completing this from. Submission of this form constitutes an agreement with the terms outlined in the Senior Exhibition Policy. Enter information EXACTLY as it is to appear on wall labels. A reasonable monetary value must be given for each work you plan to exhibit even if the work isn't for sale. Submission of this form, and delivery of art work must be in accordance with the times and directions specified in the exhibition schedule, otherwise, credit for your participation in the Senior Exhibition may be withheld.

You may exhibit up to five pieces but the total width of your work cannot exceed 96 inches.
