Clinical Experience in Puerto Rico

Traveling Abroad?

Getting ready to visit friends over seas? Taking you first business trip to a foreign country? Fed up with it all, and headed for that tropical desert isle? Whatever the genesis of your trip destination, if you are preparing to travel overseas, you’ll need to secure a passport.

Complete the application

You may pick up the passport application (FORM DS-11) at any US Post Office that accepts and processes passport applications or log on to www.aaa.com (Click on “Travel” then select “Traveler’s Tools” to print out a passport application. After completing the application, you will also need to:

Present proof of US Citizenship

You may use a pervious passport, a certified birth certificate (with a raised, embossed, impressed, or multicolored seal, the registrar’s signature, and the date the certificate was filed, which must have been within one year of your birth – photocopies and hospital certificates are NOT acceptable), a Certificate of Citizenship or Naturalization, or a Consular Report of Birth or
Certification of Birth.

Present proof of identity

For most a valid permanent state driver’s license, a Government ID of Military ID will suffice. A social security card is not sufficient to prove your identity.

Provide two passport photos

Passport photos must be 2” by 2” and identical. They can be black & white or color and must have been taken within the last 6 months and show your current appearance, full-face looking directly at the camera. One of the many services AAA East Penn provides its members is passport photo service – no appointment required – special member price: &7.95 per set.

Pay the fees

For anyone 16 years of age of older, the passport processing fee is $55, along with a fee of $30 for the execution of the application. Your passport will be valid for 10 years from the date of issue except where limited by the Secretary of State. Young people age 15 and under must pay $40 in processing fees, plus $30 for the execution of the application. (NOTE: A child of any age must be present.) Your passport will be valid for five years except where limited.

Provide your SS#

If you do not provide your Social Security Number, the International Revenue Service may impose a $500 penalty. If you have questions about this, please call your nearest IRS office.

One last thing…

Do not sign the application form until the Passport clerk tells you to do so. The Passport clerk must witness you signing the form!
