Clearance Requirements for Practicum and Internship

• FBI Fingerprint Clearance information available at: www.pa.cogentid.com. The fee is $40.00. Follow the directions on where to go and how to schedule.
• PA State Criminal Clearance can be found at: https://epatch.state.pa.us/Home.jsp. The fee is $10. This is done on-line so you get results instantly, just be sure to print out the results.
• PA Child Abuse History Clearance. The form is available at: http://www.dpw.state.pa.us/ucmprd/groups/webcontent/documents/form/s_001762.pdf    This form must be mailed in with a money order only.
• TB Test. The fee is $10. This can be done at the Health Center on campus or by your own physician. The dates at the Health Center haven't been determined yet.
