REQUEST FOR AN EXEMPTION OF A MEAL PLAN


A. Purpose

The purpose of this policy is for residence hall students requesting exemption or modification of their required meal plan due to medical reasons.

B. Scope

This policy applies to all students residing in residence halls where a meal plan is required.

D. Policy & Procedure(s)

 

1.       Obtain a Meal Plan Exemption form from the Food Contract Office, Stratton

          Administration Center, Room 219.

2.       The form must be completed by your physician.

3.       Return the completed form to the Food Contract Office, Stratton Administration

          Center, Room 219, P.O. Box 730, Kutztown, PA 19530-0730

4.       The Meal Plan Exemption request will be reviewed by the Health Center Director,

          the General Manager of Dining Services, and the Food Contract Office Assistant Director. 

          If it is determined that Dining Services is unable to meet the needs of your particular request,  you will be exempt from participating in a meal plan.
