Disability Services - Textbooks in an Alternative Format

The form below is for current students who have been declared eligible by the Disability Services Office for the reasonable accommodation of textbooks in an alternative format. Please provide the information requested on the form. The office will order the texts from the Recordings for the Blind and Dyslexic, Inc. (RFBD) and will inform you when they are received or if they are not available. If you are submitting more than five requests, press the submit button and return to this page and complete additional forms.
