Disability Services - Request Note Takers for Your Classes

The form below is for current students who have been declared eligible by the Disability Services Office for the reasonable accommodation of having note takers provided to you for your classes. Please provide the information requested on the form.

*Please check for your notes in the Disability Office in Stratton Administration Building in room #215 by the end of the second week of the beginning of each semester.
