Disability Services - Request Note Takers for Your Classes

The form below is for current students who have been declared eligible by the Disability Services Office for the reasonable accommodation of having note takers provided to you for your classes. Please provide the information requested on the form to be request notes for your classes.

Some time after you have requested notes, you will be sent instructions along with your individual user name and password from, Mr. Troxel GA via the notetakers@kutztown.edu email address.

You may then begin checking on D2L ( https://desire2learn.kutztown.edu/ ) for your notes.

**If you withdraw from a class please notify the Disabilities Office ASAP (as soon as possible), by emailing mclaughl@kutztown.edu.
