Incident Report

An Incident Report is the sole vehicle by which a residence life staff member makes his/her observations known to the Central Office and Dean for Student Services and Campus Life. The observation should indicate a violation of university regulation has possibly been committed by one or several individuals. Incident reports represent concise and precise remembered observations regarding a specific event or set of circumstances. INCIDENT REPORTS ARE LIMITED TO FACTS AND ARE NOT TO INCLUDE OPINION, HYPOTHESIS, or guessing. Accurate and comprehensive relevant information must be included.

CONTENTS OF THE INCIDENT REPORT FORM

  1. The name, campus address, sex and student ID number of all individuals involved.
      Student ID numbers are obtained from (1) the Student ID Card, (2) the resident director/residence coordinator
  2. The degree of cooperation:
    1. Compliant = cooperative
    2. Argumentative = uncooperative
    3. Downright nasty and abusive = hostile
  3. Date of Incident
  4. Time of Incident
  5. Location of Incident
    1. Identifiable to the nearest room
    2. Building name, floor number, wing, room number
  6. Violation
    1. The violation is stated, not the event
    2. The violation is specifically applied to the person
  7. Type of Evidence (damage) observed and what was done with the confiscated evidence
    1. Information is specific
    2. The evidence is given to the RD/RC for safekeeping, due to the fact that it may be necessary for a hearing.
  8. Description of Situation
    1. Is brief but precise
    2. Events are placed in chronological order
    3. Hearsay or opinion are not included that may discredit some of the testimony
  9. Witnesses
    1. The names, addresses and student ID numbers of anyone else who observed the same events are submitted.
    2. Additional details observed by others are generated into incident reports by them not the person submitting the report.
  10. Name of person who submitted the incident report.

ADDITIONAL COMMENTS:

  1. The text under “Situation” should be specific. The report should not say “Violator A was disorderly”. It should say "Violator A called me a 'rotten bastard' and then slammed the door in my face".
  2. If profanity or abusive language was used, this will be indicated.
  3. As much as possible, individuals are informed that they are being written-up.
  4. If more than one person is involved, the sequence of events and behavior are related to specific individuals.
  5. ACTIONS AND BEHAVIOR ARE REPORTED.
