All full-time and part-time undergraduate students who are matriculated in a degree program are required to complete the following Required Medical Forms (RMF) through the website, My Health Link.  To access the portal, students must use their NETWORK USERNAME AND PASSWORD, which is issued as soon as a student has been charged the advance registration deposit.  If you cannot locate or remember this information, contact the IT Help Desk at 610-683-1511.

• Electronic Health History form
• Electronic Insurance form (only if you are insured)
• Electronic Notice of Privacy Practices form
• Electronic Permission for Treatment Agreement (18 years or older) OR
• Permission for Treatment Agreement for Minor (parent/guardian must download, print and submit)
• Immunization form (mailed to students)
• Electronic Options for Meningitis Compliance (required for resident students)

Completion of the required medical forms must be done whether a student resides on or off-campus and regardless of intent to utilize Clinical Services.  MAC users must download Microsoft Silverlight to access the site.  FAILURE TO COMPLETE THIS INFORMATION WILL LIMIT YOUR ABILITY TO REGISTER FOR FUTURE COURSEWORK OR VIEW YOUR TRANSCRIPT.

INTERNATIONAL & EXCHANGE STUDENTS

All exchange students (J-1) attending Kutztown University must complete the Required Medical Forms (RMF) through the My Health Link portal.   MAC users must download Microsoft Silverlight to access the site.  Proof of two Measles/mumps/rubella (MMR) vaccines and a tetanus vaccine within 10 years must be documented.  Students who are missing immunizations will be required to obtain the necessary vaccines at their own expense through the university clinic. 

International students (F1) attending Kutztown University must complete the Required Medical Forms through My Health Link. MAC users must download Microsoft Silverlight to access the site.  The following diseases will require proof of immunity by vaccine, titer, or physician-documented illness:  Rubeola Measles, Mumps, Rubella (German Measles), Hepatitis B, and Chickenpox. A tetanus vaccine within the past 10 years also is required.  Students in high risk countries for tuberculosis will be asked to complete a screening questionnaire.  Any missing vaccines or titers must be obtained as soon as possible during the first semester of attendance. 

GRADUATE, CERTIFICATION, PROFESSIONAL CREDIT, LIFELONG LEARNING AND ON-LINE PROGRAM STUDENTS

Students must complete one of the two options outlined below.  

1.  RMF WAIVER FORM (download here) - select this option to waive the Required Medical Forms (RMF) i.  This form does not waive the semester health center fee if it has been assessed and must be paid whether or not you plan to use Clinical Services.  If you subsequently utilize the clinic or reside on campus, this form becomes void and you will be required to complete the Required Medical Forms.  Students enrolled in on-line courses will be required to complete RMF upon registering for an on-campus course.

2.  REQUIRED MEDICAL FORMS (RMF) - select this option if you plan to access healthcare or TB testing through Clinical Services, purchase the university health insurance plan or reside on campus.  The RMF can be completed through My Health Link.  MAC users must download Microsoft Silverlight to access the site.  In addition to completing these forms on line, you are required to submit a copy of your immunization record to Health Administrative Services.  This information also is located on My Health Link.
