Insurance

In the event that a student-athlete is injured while participating in a "Covered Sport", it is important for our office to know about any medical coverage they may have in order to avoid delays in injury management and in claims processing.  Therefore, as a requirement for any level of participation, all student-athletes must complete a "Athletic Insurance Information Form."  Please note, it is vital that all requested information be completed in its entirety.  Also, any changes to your coverage must be reported and updated immediately to avoid injury or claims management problems.  The policy holder, along with the student, must sign and date the form at the bottom and return it to the Office of Sports Medicine before the student-athlete may participate in any KU athletic activities.

            Athletic Accident Insurance is provided by our institutions’ Kutztown University Student Services, Inc. (KUSSI) for the benefit of our student-athletes.  This coverage is offered on a "Excess" basis only.  Under the terms of the policy, the coverage is considered secondary payor to all other valid and collectable medical insurance policies.  Most notable would be parental insurance coverage through your place of employment under which the student-athlete is covered as an eligible dependent.

            The KU Athletic Accident Insurance is comprehensive, but it has standard limitations.  It does not cover expenses due to illness or non-athletically related injuries.  It does not cover expenses incurred from "Off-Campus non-KU Affiliated Physicians / Medical Specialists" unless a written referral, pre-approved by the University’s Team Physician and/or a Sports Medicine Staff member, is obtained.  It covers only "In-Network" Providers, unless sent by a member of the Sports Medicine Staff.  All surgical treatments must have prior written authorization. Only those expenses incurred within a period of two(2) years from the date of the injury are covered.  Catastrophic insurance is available through the NCAA and must be initiated within the same two year period.  It is therefore our recommendation that each student-athlete carry personal health insurance to cover these non-covered expenses should they arise.  A student-athlete's failure to carry personal health insurance for these expenses may delay prompt non-emergency medical treatment and therefore his/her return to competitive athletics.  We suggest those student-athletes without personal health insurance look to purchase the ‘Accident & Sickness Insurance Plan’ offered through the University’s Health and Wellness Center.  You may contact them at (610) 683-4082.
