STATEMENT TO BE READ TO STUDENTS

At KU we are committed to the quality of education. To that end we periodically ask our students to evaluate the quality of our instructors and their courses. Your thoughtful, sincere, and honest answers to the questionnaire will serve two purposes: (1) helping your instructor improve his or her teaching, and (2) helping those who will make decisions about promotion and tenure for your instructor.

A word of caution about the survey questions on the twelve course objectives. No one instructor or class can cover all of them, so your instructor and his or her department have identified those that are most important for this class. Therefore, you will likely find that you won't evaluate your instructor highly on all twelve; this is to be expected, and the less important objectives will not factor into the final evaluation.

Your responses will not be seen by your instructor until after final grades for the course have been submitted to the Registrar. Your responses and comments are to be anonymous, so please do not sign your name anywhere. Please do not write in the "Comments Section" on the reverse side of the form; instead, use the extra sheet of paper you were provided for any comments you may have.
