THE TRANSCRIPT REQUEST FORM IS AVAILABLE IN "PDF" FORMAT.
TO VIEW AND PRINT IT:

1. Download Adobe Acrobat Reader (it is free).
2. Follow the directions on Adobe's download page and install it on your PC.
3. Once installed, open the Transcript Reqest Form (20kb) with acrobat reader.
4. From the Acrobat reader select "File" from the menu bar, then Print.
5. Fill out the form and mail it to the Registrar's Office.

When sending transcripts via Federal Express, UPS, Priority Mail, etc.
