Statement of Financial Interests
Faculty and staff members of the State System of Higher Education are considered to be public employees under the Pennsylvania Public Official and Employee Ethics Act, Chapter 11, 65 Pa.C.S. § 1101 et seq.
The “Ethics Act” prohibits public officials and public employees from engaging in activities that involve the use by a public official or public employee of his/her office or employment or any confidential information received through his/her holding public office or employment for the private pecuniary benefit of him/herself, a member of his/her immediate family, or a business with which he/she or a member of his/her family is associated.

How to submit completed forms to Human Resources:

Option 1: You have Adobe and you are able to use its electronic signature function
Sign your SFI as a PDF with an electronic signature. Email the signed form to human resources at firstname.lastname@example.org.
Option 2: You have a scanner and printer at home
Print out the SFI form, sign it, scan it and email the scan to email@example.com.
Option 3: You have access to email but not Adobe digital signature
Complete your SFI form, attach it to an email and email it from your university email address to firstname.lastname@example.org with the following statement copied into the body of the email:
“I affirm that the attached information is true and correct to the best of my knowledge, information, and belief. Said affirmation being made subject to the penalties prescribed by 18 Pa.C.S. § 4904 (unsworn falsification to authorities) and the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1109(b).”
When we are permitted to return to campus again, you will need to provide your signed SFI to my office within 14 days of the date you return to work.
